You can use this service if you:
- want to register your child at the surgery
- live within our catchment area
- are registering your child
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- the details of the child you are completing the form on behalf of
- your child’s NHS number
- details of previous GP surgery
- basic health and medical information
You can also get a paper form from the surgery.
